Declarative - obj + had + been + V3
           A. I had written a book.
           P. A book had been written by me.
Yes/no - had + obj + been + V3
           A. Had I written a book ?
           P. Had a book been written by me ?
W.h. word question -
w.h. word + had + obj + been + V3
      A. Who had written a book ?
      P. By whom had a book been written ?
Negative -
       obj + had + not + been + V3
      A. I had not written a book.
      P. A book had not been written by me.
